Gas explosion sparks crucial need for proper electrofusion training

A recent explosion at Cadia Mine in New South Wales, Australia has sent shockwaves through the pipeline industry, emphasising the critical importance of proper training and adherence to safety protocols in electrofusion welding.

On July 22, 2024, two workers were extending a high-density polyethylene (HDPE) pipeline in a tunnel when an electrofusion welding process went awry.

The incident, which scattered debris over 60 metres, occurred shortly after the workers had completed the welding and left the area.

Investigators found that the workers had used abrasive sanding pads and paper towels to clean pipe ends, a practice that likely contributed to the explosion.

This method of preparation falls far short of industry standards, which require mechanical peeling to remove the oxidised layer and ensure a clean surface for fusion.
